I've had various brands of unilocs for ~15 years. The only time I've had one come loose was when either the joint pin or shaft insert were dirty or had debris in them. The weight bolt would be my first guess. If it's not that, it's probably a manufacturer defect in the butt or the shaft.

If you see a silver label Mali, some of those are 5/16x14. For a new one, you can get one from PoolDawg for $90. I had one of those in a radial pin and it was decent. Certainly worth the $90.
