I think if I put up % of why I did not run out a rack, I'd list

40% miss a shot I lined up for
25% hooked myself - often by 1mm
20% misplayed shape and missed a tough shot or made it but no shape on next ball which ends up in some good shots but most often a loss
15% messed up a safe
Don't forget dry breaks, unless you're only counting times you're at the table shooting after the break, or you're playing 9 ball with a magic rack. :)

Yeah I fall under I beat myself and tend to give games away easy category.

You miss shots and lose games. What is questionable about that?
If you miss more than your opponent... you lose. Simple.
Obviously, you have to do something about it. Change how you play. Get better and get smarter.

A dry break is a bit out of your control, as is having a decent shot after the break. I was thinking of situations where you are already at the table having a grand time running out and you do something to make yourself lose. I also did not add in scratches as I've seen some way odd scratch situations (you make a tough cut and cue ball hits off 3 other balls and falls in) and really that would be almost a "bad position" loss.
