Break ball. What is considered being "In the rack"?

When you rack the balls is a break ball considered in the rack if it touches the wood rack and not the balls? The wood rack is 1/2" thick and there have been times this has happened. I have a ball marker that can be used to move the ball and place it back after the rack is removed from the balls. Or if it touches the rack proper do you spot it? Thanks ahead of time.
 
If the rack touches it then its in the rack and spotted on the headspot. If the cue ball happens to be on or to near the headspot to cleanly place it on the headspot then it goes to the center spot (between the 2 side pockets).

I the cue ball is in the rack it is ball in hand behind the line.
 
Thanks everyone. I was clear on the Cue ball but was hoping being in the rack meant touching the balls. Would have been nice last night with the easy bump break shot on the front right corner ball.

it is unless the 15th ball is in the kitchen, then CB goes on the head spot and doesn't have to cross the line. if the CB is in the rack and the 15th ball is on the head spot, then the CB goes in the center spot. if both the 15th and CB are in the rack, then the 15th ball is on the foot spot (full rack) and CB is in kitchen.

doesn't happen that often.
