Does anyone know if one is available via online PDF? What I really need is the fastener list which appears, IIRC, in the back section of the document.

Thanks Maxx. Just what I needed. Evidently I was mistaken as fastener size, description, and quantity accompany the installation prose and are not separately listed. Very nice find, thanks again!

Rick, glad it did the trick. You should thank AZ's very own Mr. Bond for the document. There is a lot of interesting stuff at the Chicago Billiard Museum.
