How do I attach a phenolic cue tip?

I need help. Last year at a tournament i was in, i had a cue retailer/ maker who also put tips on cues put a phenolic break tip on my break stick. Just recently the tip just flew off when i broke. So i tryed sanding the tip and ferrule and using super glue to attach the tip. It stayed on for one break and flew off the next. I tried it again with more glue, but it still fell off. Please help.... Is there a certain kind of glue i need to use? If so, please tell me what it is. I dont have any cue makers/repairers to ask or do it for me.

You might try the ask a cue maker section for more (correct) answers, but aside from ensuring both sides are flat and using Loctite gel adhesive I recall somebody saying that scoring the glue side of the tip helps.
