Does anyone recognize this make/model table?

What shape is the nameplate? Are the rail fasteners bolts or do they come off as nuts and washers.What do the tops of the legs/brackets look like?

I was able to get a couple more pics. Not sure how the rails are fastened. I'll try to find out, if only because my friend needs to disassemble/transport/reassemble the table.

Yes, once we got the plaque image we were able to find the vendor. I emailed them asking what model, the construction specs, etc.

A friend is getting the table from his dad, next move is to find someone who can disassemble/transport/re-assemble the table about 40 miles, new cloth needed of course, etc.

Table is in Phoenixville, PA and needs to be moved to Newark, DE.

Anyone have any leads on experienced table workers? We want the table to have standard cloth (not the thick slow stuff, not the slick ultra-fast stuff either). And the table needs to be as level as possible. :)
