Last time I was in a public restroom, with a sign like that, I waited and waited but no employee showed up to wash my hands. I finally just washed them myself.

I learned from my friend from New Jersey that when you see them walking out of the restroom past the sind sink to say "Hey Man!" And they stop and turn around you say "you forgot to wash your hands".... then you can say "you must have been daydreaming and were distracted, I do that all the time"... or instead they just tell you to "piss off" then you can shout out a at them "you are a filthy pig!" It's fun and usually doesn't end in a fist fight.
