Tournament Forms!?

Gonna start doing some "home tournaments" with a group of 15-20 friends. Probably a $5 or $10 entry fee, with 75% of proceeds to first place and the other 25% to second place. Hehe, maybe I should make a percentage for the house!

I'm wanting to run it double elimination style, and curious if anyone has any forms? I'd prefer raw computer forms in Excel, Word, etc. that I could modify, but at this point I would accept a scanned copy of a real form, and then fiddle with it to make my own later. Just need something on a pinch.

We use the Playpool charts all the time. BCA has some other charts you can use too.

There are interactive .PDF file forms at Dave's Pool Tools and all print on standard 8.5 x 11. You can fill them in by hand or update them on your computer and print them.
