Threaded Back Plates Chuck

I need threaded M16*1.5 back plate for my 4" chuck.
anyone knows who can make this custom back plate for me?

Is it just a 16x1.5 thread or is there a shoulder on the spindle?
Do you have a picture of the spindle/ headstock.
Are you going to drill the holes to mount the chuck?
What kind of chuck?

I have a old back plate mounting with this thread for my spindle lathe I machined it for my chuck wiht three hole but I do bad job
and now headstock have a important runout.

Is that by chance a wood lathe? If it is it probably came with a backing plate from the factory. If you find a machine shop they will be able to drill the holes accurately for the chuck then the problem is solved.
