I bought this in used condition as you see it here with intentions to put to good use.
Unfortunately do not have any spare time currently or in the near future!
It comes with everything pictured less lathe and DVD (instructional) from Cuesmith.com

I am in Arizona and I have a deluxe lathe.
I believe it will fit but, might check with Chris Hightower for verification.

Show a picture of the end plates fromt eh end view that lock it to the bed and I will let you know if it fits.
 
No taper bars.
I just blew up one of the pictures. It will work on either a mid size or deluxe without any major mods. The two end plates will just need the risers taken off and the plates turned around and the knobs put in them. Less than 5 minutes of work.

I am not sure what I said to make you pass up this good of a deal, because what I posted says everything is good for this attachment on either height lathe with like five minutes of one time set up. His price of $850 is almost half what it would be new at $1595. These usually sell for between $1100 to $1200 used for a later model like this one.

I just have not made up my mind if I want to do inlays or not.
