Contact Brian Russell, of Billiards Revival. He is based in Newburgh, NY. I think that he has a couple of IV's.

Also try Oscar, at PoolTable911. He's based in West Creek, NJ. They usually have a pretty expansive inventory.

Both are reputable and conscientious installers.
 
Trent McRitchie may still travel between OH and Maine.
Karl Sloezen in NJ does a lot of Brunswick tables. Example listing is below. Should be just a matter of time before he has a IV if not already.
