I've had various brands of unilocs for ~15 years. The only time I've had one come loose was when either the joint pin or shaft insert were dirty or had debris in them. The weight bolt would be my first guess. If it's not that, it's probably a manufacturer defect in the butt or the shaft.

I threw a McDermott butt and a Revo shaft in the 1x1. So long as you insert the butt bumper down and the shaft tip down, it has a pretty snug fit.
