There are a few different ways to run triple elimination tournaments. None are too simple.

Another one that I like is that you don't use a bracket at all. You redraw match-ups every round. Say you have 16 players. You draw match-ups and play the 8 matches. Then you redraw the 16 players again and play them. At this point it's possible that some players have 2 loses. You repeat this again and again. One key thing to note is sometimes you will have players that will get a bye in that round. You play down to the final 2 players and the finals are determined by how many matches each player has lost.
