Even a slight uneven will make the table roll off right?

lets say the level is suppose to be this

| |-O-| | even
| |--O| | off to the right a little

How do you measure... middle table, diagonal, etc...

Quite the vag question. But to level a table it's best to use a machinists level (very accurate) if it's 3 piece slate make sure the frame is level first then start with the middle section. Then butt up your seems making them flush (very flush). Then level the outer edges while keeping the seems flush (again; very flush) then fill the cracks with beeswax or bondo (make sure smooth) Diagonal isn't nessasary, but left to right and 90 degrees off left to right

Hope this helps!
