When I do one of these, I bore it out, no sense in heating it to remove it seeing you are going to plug it anyway and start over.

That's one of the reasons, I use brass plugs and do the drill bore and tap after it's in the shaft. If it's the case of a quick release uni-loc, I plug it with phenolic instead of wood.

I've had really good luck with the cnc'd self-aligning inserts. They go in straight every time.
