G Core shaft

Anyone have any trouble putting a tip on a g core shaft? I’ve had 2 pop off. After the first one popped I switched glue(to loctite proffessional) and replaced the .030 fiber tip pad . Got a text today and it came off again. Been doing this more than 20 years and I can only remember 1 other tip coming off.
 
We've got 3 of them. Have put various Ultraskins on them and never had one come off. I don't play with them much anymore though. I use Loctite also. While not a professional cue guy, I do have a cue lathe and have probably put 75 or so tips on, mostly Ultraskins, or lately some Triangles, and have only one come off. Turned out the ferrule was loose and I didn't catch it.
Maybe they are miscuing a lot or possibly not taking care of the cue and it is falling down on a concrete floor. Our Monday night league with 48 players and concrete floors, I hear cues fall and slap the floor 2 or 3 times every night. My teammate knocked mine down one night and took a bit to get the embedded grit out of the shaft. I don't stand mine next to hers anymore.

Hope that's not for Triangles, or Ultraskins for that matter. I'm down on my table shooting a few shots with them in an hr or so. I don't do draw or break shots till the next day though. May have to change my ways.
 
It’s an Ultraskin ivory…. I’ll check the ferrule closely when I get it back tonight, maybe it’s loose.
 
The one I had come off because of a loose ferrule, was an older inexpensive cue with a non capped ferrule. If I remember, my Gcores are basically a capped version, I think, but it's been awhile.
 
The first failure was between the tip and the pad. I replaced the pad thinking it may have been the problem and was getting thin anyway. I’ll get the shaft tonight to see where it failed this time.
