How bad is my form?

Recently I started feeling like something wasn't quite right with my stroke. I felt like there was some lateral movement in my backstroke. I would usually notice it in my first warm up stroke, then correct for the shot....at least I thought.

Last night I decided to try taking a video from behind me with my phone to see what was going on. I was shocked to see how crooked my back arm was. I have attached a pic to show what I'm talking about.

After I saw it, I tried shooting the rest of the night with my back arm as vertical as possible, but it felt very uncomfortable and I was dogging a lot of my shots.

I shoot okay. I'm an APA 6. I've been progressing ever since I started playing league almost two years ago. I can roll it pretty straight most of the time. I don't always notice the sideways movement, but I also feel like I setup on the ball the same each time. Would I have to be compensating for this in my wrist during the stroke?

Do any of you shoot successfully with a similar form? Is this something I must change immediately to continue getting better?

The movement you feel uncomfortable with is your body trying to adapt bad habits to new knowledge. Plenty of people shoot real well without having to have their forearm perfectly vertical. I wouldn't worry about what your back arm is doing, more so that your head is aligned properly over your cue.
 
Kind of difficult to see here as there isn't much to reference, like where the shot line is, and I'm certainly no expert, however, it appears that your stance/alignment might be off. Looking just at the butt, it appears that the cue is not on a straight line but it could just be the angle of the photo.
