name plate replacement

I need a replacement name plate for my pool table. At this point I am interested in filling the opening which is 4 1/16 length and 2" wide with rounded corners. I would be OK with someone engraving or purchasing one already engraved as the opening looks horrible.
 
You may be able to get what your looking for at most Trophy and Award shops locally.

Try contacting the manufacture. I go a new one on my table right from a e Schmidt for $10. wasn't as old as the table (1951) but new was better than nothing.
