Is it correct that the rules for the opening break (pocket a ball or send the cue and two object balls to a rail) *only* apply for the opening break, and not for subsequent "breaks" of the 14-ball re-racked balls? In other words, for *all* shots other than the opening break the standard rules apply, i.e., pocket a ball or hit an object ball and drive either the cue ball or the object ball to a rail?
 
You are correct that it only applies to the opening break. Then after just a legal hit with driving a object ball to a rail or after contact the cue ball to a rail, or pocketing a ball. The exception is after a 3 foul rule infraction, then the balls are re-racked and the opening break rule is on again for the player that 3 fouled.

Here are the rules. I think it is useful for all players to read through them at least once.
