What are some thoughts about a tie breaker game for team tournaments in 8-Ball.

I think it would be interesting to have a scotch game for the final game to determine the winner. Scotch Doubles, Triples, Quads or Quints.

A team could select 2 or more players instead of one for the decider.

Would it be too confusing for many teams, especially for those players who have not played in a scotch doubles event?

Too much of a gimmick?

I don't know how often a tie breaker game comes up in a points format.

Many leagues have ways to break ties. The most common is probably picking one player from each team for one game.

I like your idea of a "scotch quintuples" play-off game for 5-person teams.
 
The tournaments we have been going to lately are 5 person with 3 B's, 1 BB, 1 A. The B's play a race to 2 - 1 game each maximum of 3 total games.
 
Scotch quintuples seems like it could be fun. I'd enforce that the order would stay the same as the order of the starters. So you could look at either scoresheet to know who shoots next. I'd also consider using a shot clock if you allow talking, especially if it's more than one game.
