What is it that makes the top players fall off?

I think it is several things and many have already hit on many of them.

One thing for sure no one can escape aging (unless of course they die). I know personally my eyes aren't half what they once were so glasses are needed, physically I don't have the stamina to play for hours upon hours, and my reflexes certainly aren't what they were 40 years ago. Age catches up with us all, providing we keep breathing.

Don't believe me, if you are over 40 go try and do the physical things you used to do when you were 18.....you won't be able to get out of bed for days........if your 60 go try to do the things you did when you were 40.....same thing. The mind may tell you it's possible, but the body revolts. All of these and the added fact that many times as you age, your priorities and things that motivate you change in ways you would never have imagined.

When it comes to comparing younger players to older players of approximately equal skill:

Younger players have lots of energy and seem to want to spend more time aggressively going for the win.
Older players have lots of experience and play a more tactical game.

I'm an old geezer and apply all sorts of tactical tricks to offset a younger more aggressive player. If I played the pace of youngsters, I would lose a lot more games.

It's all about concentration & intensity. As you grow older it's much harder to keep the intensity in your concentration.
