wood out of round

I have had a couple of shafts get out of round when I am doing my final sanding. What causes this? They seem to be turning true when I start sanding them and then I can feel a slight out of round shape. Am I putting too much pressure on them when I am sanding or doing something else wrong? Any suggestions would help.

P.S. I start with them slightly oversize and put the ferrule on. I turn the ferrule down to size and the sand the wood to match.

Sanding with the lathe spinning in opossite rotation about halfway through should help.
