Who decides the 31 countries? Who chooses the two players?

My guess is Matchroom sports. I know a few teams qualified through tournaments, but the majority are probably selected by the guys that produce the tourney.

I think team Philippines was selected by WPA ranking at the time. Which was Warren Kiamco and Biado. Kinda surprise not to see Efren/Busty/Dennis/LeeVan, which were previous winners of the World Cup.

Matchroom. It is a private promoted made for TV event. Too many people want to ascribe Olympic like notions to these Matchroom events. "It isn't fair England has two teams", "It is Wales not England", blah blah blah.
