Definitely an old Adam but I looked in the blue book and couldn't find the model, maybe a custom but I'm sure Jayman will be much more of a help, he is the expert on Adam cues
 
As stated above, it is indeed a 70's Adam. The forearm is a standard Adam blank used in many of their cues, The butt-sleeve design was used in very few cues and was never offered in any of their catalogs as far as I know. That cue was most likely used as a salesman's sample to demonstrate the Adam style and quality to potential buyers.
