Has anyone seen a game where one of the player exceeded the extension time or just cue right after the end of the extension time? What happen in such a scenario?

If it is just a foul and the player has not played his stroke yet, that is fine. But what if he has struck the cue ball and disturbed a bunch of balls? All to be returned back to their location prior to the cue ball being played?
In 3C billiards I think the incoming player gets the break shot. Since top level players rarely miss the break shot it is a pretty significant consequence.
