Have you disassembled your Chuck lately for cleaning purposes? If so, you probably have your jaws misaligned. Just keep opening the jaws all the way and reposition the order in which they start to thread.

I do. I have records stored in the computer and a print out in a binder. I take pics of the cues in stages and either send them to the customer or post them on the site.

Correct, it has everything to do with preparation and installation. I had 1 customer that his came off. I inspected the Ferrule to assure correct facing and called Samsara to see what they recommend for adhesive. It wasn't glue... They use a 2 part epoxy. 420. Sold by Atlas Billiards. Never had...
