Plain bad luck for the shooter. The ball has to be hit perfect for it come back out. On tables without a ball return I've always cleared the pocket of all balls except for one to keep the pocketed ball from looping around and back out.

Be careful because in some places call shot and call pocket are two different things. In call pocket it doesn't matter how the ball gets to the hole as long it was done with a legal hit on your object ball. Call shot to some it means that you need to declare kisses and banks. I've seen...

Depends on the rules of the game. Open after the break for me maybe 5 minutes or less. Take what you make on the break 9 minutes or less. Maybe more if the table is breaking slow. (more safeties).

Maybe, depending on what type of cue ball we're playing with, I might consider cutting the eleven ball and drawing around three rails for the nine ball. If I fall short of the nine ball I will option to get back to it having position on both the ten and twelve balls.
