Found it. If you look at the top of the windows in the Shorpy image, http://www.shorpy.com/node/9841?size=_original#caption You can read that it says, Union Billiard Academy. If you search for Union Billiard Academy, there is an article that says it was located on Broadway at 45th St.

Hey all. I just picked up an older table made by United Billiards Inc. Its a typical "bar box" style, slate bed, coin operated, 7 foot table. Kind of resembles any Valley table that you might see in a bar room. It seems well constructed. I have just never heard of that manufacturer before...
