This has been covered in several other posts a while back. There was a bad batch of "hybrid" tips that the BK Rush had installed. These tips were the same as the Samsara "hybrid" break tips. Apparently, the resin infusion into the leather-layered tips didn't hold well and the tips would separate...

I'm certain if you cut that tip off and send it back to Bulletproof for examination they'll send you a few replacements of your choice.
