5 person team, race to 13 score sheet for download?

I am looking for the format the BCA uses for 5 person teams. 5 rounds with the visiting team rotating.

Does anyone have one I can use so I don't have to draw one up from scratch?

Thanks for looking for me Shannon. Those are all handicapped league night score sheets. I'm looking for a straight race to 13 non handicapped sheet like you would use at the BCA national team event.

Just play even with no handicap... The sheet is all setup in rounds and shows who breaks..
