Breaking from the rail, aiming for the second ball, on medium strong break shot, I noticed my cue ball flies a bit then hits the side rail, on strong break shots, does same but after hitting the side rail, it flies off the table. How do I prevent this from happening?

The cue ball flies off the table because it is not on the table when it hits the rack. Probably, you need a more level stroke.
It's also possible that the cue ball hits the rack after the first bounce on the cloth on the way to the rack. Maybe you can tell from the skid marks along the path to the rack. If so, a little more elevation will cause the cue ball to land just as it gets to the rack. Or, figure out how to speed up the cue ball to get the same result.
