I have done some work for moving companys and they will hire a mechanic to creat your table for shipping. When at new location they use a local mechanic to unpack and reassemble table. This may be an option for you, guess it's all about how much you love that table.

Unless your really want to buy another table for your new place, your best bet is to either hire a mechanic there to tear the table down and crate it up for shipping (Or Do it your self like I did for my Gold Crown II).
Then hire another mechanic to set it up at the new place. I would start looking for them now. if you want to to know how I built my crates for my slates PM me.
