Does anyone know Mark Levinson of Kentucky

I have a cue that I got recently and I was getting it ready for Cory to refinish, and when I pulled the wrap off it, it had the following info under the wrap:

Mark A. Levinson
7-11-92
and a phone number for Hazard, KY (not the correct one anymore)

I wanted to try to get in touch with him and get some info on the cue, if possible.

If anyone knows him, please PM me with any info/contact info. Thanks...
