The TD is not there to be fair to both players, they are there to enforce the rules (that they should know) which typically comes with being "unfair" to the one that has the rule enforced against.

The rules answer your question as to how to spot it and you attempting to bring uncertainty into that by saying it has to be sitting exactly as it was before it fell by using the word "accurately", is irrelevant. You put it as close to the spot as you can and that is it, nothing more nothing less.

Who came up with the five second rule?

The rule has been around for a while. Pat Fleming suggested the 5 part while it had been 3 before.

The 1977 BCA rule book does not have the rule. In the 1980 rule book this is on page 53:
