I think everybody has on multiple occasions. However, the
great players do it FAR less frequently, and if you think you
could use improvement in this area, today I wrote a blog post

There could be several reasons, the most commonly blamed

You may be on to something because I can't think of the last time I missed a ball when I didn't have to play position but needed to put at least some thought into where the CB was going to avoid a scratch.
