I call people Mr, Sir, by their first name, by various other names, so forth and so on. Don't think it is pretentious at all. In a polite society you don't address someone by their first name unless you know them after all.

You can NEVER go wrong with "Mr." or "Sir". Most individuals will tell you to call them something else if they have an issue with it...or ask you to address them in that manner if it's what they prefer. I don't think it's a big deal regardless.
