I agree, had a couple happen to me....changed a few things and so far has not happened since7/16 20. Too fine and too short for wood imo.
Make sure you grind two flats on those inserts . They are infamous for spinning out .

Live threading with my Joey cutter gives those type of threads a hell of a lot better chance than trying to use a tap which IMO would be futile at best.7/16 20. Too fine and too short for wood imo.
