Assuming your hitting the CB at the same spot, what is the difference between these two types of shot?

The left one is using parallel English keeping the entire cue parallel to the intended shot line.
The right one is using "backhand" English where the cue is aligned across the intended shot line.

Do you adjust the same for deflection?

Both deflect, but you adjust differently.
With the left image, you adjust your aim.
The idea behind using BHE (right image) is to line up str8 and then pivot the cue to your desired english. The pivot takes care of any adjustments needed.
Using the swiping across method that aims up str8 and then dynamically arrives at the right image, you make your adjustments with feel in your hands... a technique that has largely fallen out of favor but has been used with great success by many pros in the pre-LD shaft era.

The tip of the shaft hits the same spot on the CB

Yes, but the cue's angle of approach on impact is different for each shot.

The shot on the left will deflect the cb more to the right (referencing the shot line), when compared to the shot on the right.

I tend to shoot using left hand english as indicated by your left pic on close, slowly hit shots. On longer, medium to hard hit shots I do the right pic (BHE). Everything in between gets a combo of both.

The cue balls will go in different directions. Is that a problem? They will also have different speeds and spins.
