As a customer I would answer your question with the following items as top 5. And I don't think it matters which are first etc. I would send the questionaire to a customer after the install not give it to them at the install where they may feel pressured to answer positively. Or send it to them via email where they can do it without having to spend 44 cents on snail mail..
