The American Legion at 1251 Ring Rd Elizabethtown, KY 42701. They have two 8-foot tables.
Hot Shot Vapes and Smoke Shop 1006 N Mulberry St, Elizabethtown, KY 42701. They have two 9-foot tables (may have three 9-footers now as they were supposed to be getting another). $5/hour
In Radcliff just off of the Fort Knox base there is a place called Gold Bars Billiards and Games, 881 North Dixie Blvd Radcliff, KY 40160. They have three 7-footers. Don't know their hourly rates.
In Flaherty (a bit further out from Fort Knox) there is Otter Creek, good bar food, cold beer, worn out 7-foot bar boxes. Also there is a place called Artie Rays but I haven't played there. Have been told they have decent tables.
Can't help with Louisville. Haven't played at them but have heard of these places. There is another place also but I can't get the name in my head:
