Great answers.It gets twisted for at least three reasons.
First, because of how it is coming off the spool.
Second, possibly because of how it was put on the spool.
Finally, twisting is part of the manufacturing, so it is always present.

Even though when the strands are wound together under pretty consistent tension, it unfortunately can still vary.

I didn't go into the detail, but because it is wound, tensioning devices tend to unwind it due to pressure. This causes twists between the spool and the tensioner.
