Who makes a cue that the shaft screws into the butt, like a threaded piloted insert about .375 dia?.

I owned one and played with it for about 10 years from ~1974-1984. But I don't know who made it. I eventually replaced it with a much nicer Rempe Adam cue.

I have a similar cue in the old junk pile as the shaft has a bow in it. It has a unique weight system & screw on ferrule. Sticker has MAC13 maybe the model number.

I would say that it is an early adams copy of the schmelke type cues and joints that were popular in the 70's.
