Painting the rails

I've installed new rail sights on the table I've been 'restoring' ( I use that term very loosely). The table is antique and has not been taken very good care of. It's had multiple coats of paint, so I'm gonna go back with some more. I've sanded the top of the rails and am ready for paint. My question is, should I just paint over top of the mother of pearl rail sights and then sand them to get the paint off, or just try and paint around them as best as I can?
 
If possible pull the sights then paint.
This should save you a lot of time in the long run.
 
When I stained my rails I painted the ivory sites I put in with shellac... Stain wiped right off... Not sure about paint....
