phenolic tip question

I had to take my phenolic tip to a guy to have it replaced and the new one popped off. I took it back and it popped off again. I'm going to do it myself now instead of wasting my time taking it to the guy again.

What type of glue do I need and any other ideas on how to install one?

The best advice I could give you and this is from all the advise I got on this subject is to have a phenolic tip with a stem on it. I took the phenolic tip that I couldn't get to stay on a shaft using all types of glue and drilled a small hole in the center not going through the tip but about half way in and then put a small screw into the tennon then cut the head of the screw off then screwed the tip onto it using loctite glue then trimmed the tip, shaped it with a file and then gave it to the customer and told him to use it hard and try to knock the tip off. So far the tip has not come off and this is about 2 to 3 weeks now. By the way thanks to all the other AZR's for all your help on this subject. All information is important in my estimation.
