1. What happens if on the last shot of a 14.1 rack you pocket the last 2 balls?

Do you rack 14 or 15?

I presume 14, but it has been so long I don't remember.

2. What happens if on the last shot of a 14.1 rack you pocket the last 2 balls and the cue ball winds up in the rack?

Rack 14 and the cue ball goes behind the line with ball in hand?

A ball whose center is not over the line, and is technically not ‘in the kitchen’, but interferes with a ball being placed exactly on the head spot. The rulebook table likely missed that possibility (I believe I had to correct mine as a result).

That's almost it. The table showing special racking cases has the line "object ball on the headspot" in the lower left corner of the table. That should be "object ball in the kitchen and obstructing the headspot". If the object ball is over the line you don't use that case.

I have never seen the case the table is wrong for. The cue ball has to be in the rack and the object ball has to be just out of the kitchen and near the headspot.
