So how is it done? Are the strands of linen wound around the cue two at a time, side by side? Or?

It is (2) strands wrapped at the same time, done the same way you install a regular linen wrap. I saw a guy doing these at a tourney one time. He generally used black along with another color and the results were very nice looking.

So the two strands would look like stripes, or would they be wound together like a candy cane?

As Dick mentioned above, it's "striped" not "twirled" like a candy cane. You can get neat designs with different color combinations. The one above looks to be black and white with black speck. I'd think you'd get a little more of a darker color if you were to use black and black with white speck. I saw a maroon and black combination one time that was very unique looking.

the idea sounds cute and imo doesn't make the cue look better than one, especially if the two linens are very different from each other. it's just different.

of course, i've always thought the choices of linens were limited anyway, so maybe this is a workaround.
