It's hard to find but not that hard.
Here is one of the reviews on Amazon....
He gave the book a mediocre rating -- applies more the the reader than the book.
Not exactly what I was looking for, but it has a lot of good information in it. I was looking for something a little more straightforward, and not as much info. I wanted templates of how to practice cue ball positioning with some basic information, but this went a little too far and intense for my liking.

I was wondering why you posted it as a today laugh because I had only read the description and saw the 29.95, thought I must be missing something. Then I went back and saw the 4995.00... lol.

Well, if someone else doesn't read the listing quite accurately and thinks $49.95 and clicks BUY IT!, the seller might get what he's after. That item has extended delivery time -- not until December. I suspect the seller doesn't actually have a copy.
The seller has nearly all positive reviews, but here are two that raise flags:

This book is a little out of my price comfort range but just in case anyone wants to know this book in gift form would complete me. 
I checked out the seller's other items and a bunch of them are the same price -- $4995. I suspect that Amazon has some kind of special treatment for items at $5k and over. You can get a very nice hoodie (Champion brand) at that price. My opinion: Amazon is complicit in the attempted scams.
Wait a minute, wait a minute... I know I picked up a copy of this somewhere along they way... give me a minute...
Ah, here it is -- first edition too! I'm rich!
