It's called an (adjustable) forkbit wrench, many billiard supply outfits sell them. The wrench has two nubs that fit into the two outer holes on the bolt head. You turn the fork wrench by slipping a common wrench onto the back end of it. The center hole in the bolt head is a screw hole to hold on an ornamental bolt cover.

Not mentioned thus far....

These are designed to fit into the jaws of a bit brace.
 
You might wanna try an automotive tool for turning in the caliper pistons on a car with rear disc brakes and the parking brake internal to the caliper. There is a tool shaped like cube and has at least 4 different pin center to center measurements and you use a 3/8" drive ratchet and extension. Im sure you could find one at your local Autozone, Advanced Auto Parts, NAPA, etc.
 
The tool I pictured is designed specifically for pool table side mount rail bolts. You can turn it with an open end wrench, crescent wrench or vise grips. The KISS principle applies. :rolleyes:
