The numbered balls represent where the CB contacts the 8 cushions. CB "A" represents where the CB lands, if it lands right on the $100 bill. I play this shot with no sidespin. I do shoot it with a lot of draw and a lot of speed. The CB bends off of the first and second rails, and must hit the third rail very close to the corner pocket (as shown) to make it past the side pocket for the fifth cushion. If you get above the side pocket, the rest of the shot is natural, and just has to be played with the right speed.
I also play almost this same shot as a 7-rail kick, pocketing a ball in the pocket nearest the starting point of the CB.
Here's the "official" diagram. I think most players will hit the first rail on the opposite side of the side pockets from where the cue ball starts. But Scott's right, rail 3 is key. Get it as close to the corner as you can.
