He used gorilla glue gel on the first one.
Then he used some type of "two part epoxy" on the rest.
IMHO there's your problem. Those glues have lots of volume, i.e. there's too much "glue space" between the tip and ferule/pad. The enormous pressure generated from a break or jump will cause a volume-glue to fail (especially if you're using english/draw -- contacting with a side of the tip rather than the center).
I had problems with losing the Samsara until I called Mueller's who recommended Tiger glue and an overnight setup before use.
Correct......I forgot to mention letting the glue set overnight before shaping...especially with the 2-part epoxies.
Tell me...what type of glue is the Tiger glue???
Maniac (likes experimenting)
Is that a TNT break cue? My buddy has one with a Purple Heart shaft and identical looking ferrule. He just got his 5th Samsara tip glued on in the past year. Maybe it is the ferrule?

Ahhh.. I thought you were installing the tips. Yeah the installer is doing something wrong..
#1 Solution = Joe Blackburn "Best in the Business"
#2 Solution = Have your installer use this and none other.
